Our partners in the WFL Division have shared some important Core Measure Education Reminders:
- SCD documentation Daily- documenting the proper device.
- Restraint documentation- make sure to chart discontinued restraints even when patients are discharged to another facility while still in the restraints.
- CHF patients need daily weights completed with 4 AM vitals.
- Education for Sepsis, CHF and Stoke documented in Meditech and the signed paper document.
- Stroke patients need repeat dysphagia screening/speech therapy prior to PO meds if they have failed dysphagia screening in the ED. NIH and vitals Q4 hours.
