Ticket to Test Reminders

Please review the attached Blood Culture Ticket to Test.

This form needs to be completed by an RN before specimen collection and signed by a nurse leader.    Additional signatures will be the RN and the phlebotomist.       

Consult your Director, Manager, or Charge Nurse for questions about when and how to complete this form.    

CLABSI and infections are monitored and reviewed.
