BCMA Reminder

As a friendly reminder, please make sure you are scanning the patient armband prior to medication administration.  If the armband is no longer scanning, please replace the armband utilizing the patient identifiers. Scanning a patient label in lieu of the armband is a violation or policy and jeopardizes patient safety.

White Boards-Defines the purpose of white boards, includes tips for better communication, and provides a visual example of a white board.

Patient Experience Pre-Round and Nurse Leader Rounding-Defines the expectations of pre-rounds (questions the nurse leader will ask the clinician) and beside validation/observation by Nurse Leader.

BCMA Scanning

BCMA Scanning Tip: Some medications may have a double bar code for scanning, upper and lower. In this scenario, the lower bar code is the bar code that should be scanned. Make sure to cover the upper bar code with something when scanning as it has been found that even though it looks as if you are scanning the lower bar code, the scanner has been picking up the upper bar code in some cases.

If the scanner picks up the upper bar code if will look like a scan did not occur.

Best Practice: Double check all medication documentation after scanning to ensure documentation accuracy.

Insulin Scanning

Insulin is billable in increments of 20 units. This includes rapid acting insulin (HumaLOG, Lispro, etc.) in addition to insulin regular, insulin NPH, and insulin glargine (Lantus).

For example:

If the patient dose is 1 to 20 units of any insulin administered subcutaneouly, only one barcode scan is required, then backspace "20" and enter the number of units to be administered.

If the patient dose is 46 units of any insulin administered subcutaneously, three barcode scans are required, then backspace "60" and enter "46".

As a friendly reminder BCMA scanning expectation is 99%. To ensure you are meeting this expectation, please print your scan rate at the completion of each shift and review with the charge nurse or designated facility contact. I have included printing instructions for review and reference; keep in mind Meditech menus can vary by Division and Facility.

BCMA scanning of Controlled Substances

Order Source: Z and W

The "z" and "w" order source in Meditech should never be used when entering an order. When either one of those sources is used, the order never goes to the physician to be electronically signed. This is particularly important for any Meditech controlled substance activity. Please comment below with questions.

Pain Medication Administration

As a reminder, please make sure you are administering pain medication per physician order. For example: Patient Smith reported pain level of 7. The physician order was Norco 2 Tablets PO Q4hours PRN for pain level 7-10 and Norco 1 tablet PO Q 4hours PRN for pain level 4-6. The correct administration for patient Smith would be Norco 2 Tablets PO Q4hours PRN for pain level 7-10. This information is included in the order on eMAR, please make sure you are reviewing prior to pain medication administration. Please comment with additional questions.
