N. Carolina Positive Patient Identification

Please review the below link for the process when a patient chart needs to be combined or has an overlay.

Combine (Merge)- Consolidating duplicate records belonging to the same patient.

Overlay- Information from two different patients is mistakenly combined under one medical record.
