Please make sure the appropriate order source is selected when entering Medications into Meditech. Medications should not be entered with the Plan of Care order source.
W – Written - order that can be found in the patient’s physical chart with a “wet ink” signature – does not go to the provider for eSignature.
V – Verbal - order taken in presence of the provider by the appropriate personnel (ex. RN) during an emergency situation or provider is scrubbed in for a procedure
T – Telephone - taken over the phone (not a TEXT) by the appropriate personnel when the provider is unable to access the EHR.
C – Plan of Care - only used by PT/OT/ST based on a previous order for Eval/Treat
P – Protocol - used when there is a hospital approved protocol, approved facility emergent standing orders, or approved clinical algorithm. - does not count against computerized provider order entry (CPOE) percentages.
Z – Dept. Process - used to carry out elements of another provider order that may depend on patient response or timing – does not go to the provider for eSignature.

NEW:Meditech Suicide Assessment Screens-Pilot San Antonio
New Suicide screens accessible from either Safety/Risk/Regulatory or BH: Suicide/Homicide Screen interventions
On page 3 of the suicide screen is a new calculated risk and the option to document a Detailed Risk Assessment
Detailed Risk Assessment can be accessed through the Suicide screen or from a stand-alone intervention
Please note this is currently being Piloted in San Antonio and changes across the enterprise are expected after the first of the year.
