Pain Medication Administration

As a reminder, please make sure you are administering pain medication per physician order. For example: Patient Smith reported pain level of 7. The physician order was Norco 2 Tablets PO Q4hours PRN for pain level 7-10 and Norco 1 tablet PO Q 4hours PRN for pain level 4-6. The correct administration for patient Smith would be Norco 2 Tablets PO Q4hours PRN for pain level 7-10. This information is included in the order on eMAR, please make sure you are reviewing prior to pain medication administration. Please comment with additional questions. Emergency Department Master Patient Index Search

C-Diff Algorithm: West Florida

Oak Hill Hospital in West Florida has implemented a C-Diff Algorithm to reduce the incidences of C-Diff. The algorithm will assist in timely and appropriate collection and should be sent to lab with all specimens. Please note, specimens should look like Type 7 on the Bristol Stool Chart and collected no later than Day 3, Day 1 being day of admission. C-Diff is considered hospital onset if specimen is collected on or after Day 4 of hospitalization. Please review provided education handouts and comment with questions.

Pain Assessment/Reassessment

Pain Assessment and Reassessment is required on each patient prior to pain medication administration and after pain medication administration. Reassessment should be completed 30 minutes after IV/IM medication administered and 1 hour after PO medication administered. Health Information Exchange (HIE)

One of the many reasons to love Meditech....Health Information Exchange allows you to view patient information outside of your market from all HCA facilities and employed doctors offices. They are currently working on being able to view information from non-HCA. This is very helpful in day to day continuity of patinet care but most recently in the hurricaines when patients are transfered to different facilities.

Each facility choses the path, examples desktop icon, link in magic key menu etc. Please verify with your facility how to access. Included is an informational resource.

CPN Education for Gulf Coast Woman Services Nurses

Park Plaza, Tomball, Northwest, and Cypress Fairbanks are presently converting from Cerner to Meditech with a staggered rollout, completion of the rollout by the end of year.

These 4 facilities also will be changing to CPN and have a few differences in the CPN documentation.  Attached is an educational document reviewing the differences. Please note this is only applicable to the 4 listed facilities above in the Gulf Coast market.

Medication Reconciliation Education

Medication Reconciliation is a requirement for completion except for OR, Dialysis and CVOR areas of specialty. Included is a presentation reviewing the process of Medication Reconciliation. All parts apply to all specialties unless included in the exceptions below.  Contact your Regional Clinical Director of Operations for questions.  Thank you each for all you do!

Exceptions:

• Rehab- Needs everything above with the addition of the appendix 1

• Nursery and NICU- Part 7, 8 and 9 only

• Dialysis is exempt from the entire med rec education

• Actual OR and CVOR areas are exempt from the entire med rec education

***Day surgery, pacu and cath lab type areas still need all of the education like the inpatient units.
